The Patient Care Navigator functions to provide access to quality care for patients diagnosed with cancer. In conjunction with the registered nurse, the Patient Care Navigator builds trusting relationships with patients through guidance, engagement, coordination of care, resource allocation, and assists with questions for the healthcare team. The Patient Care Navigator serves as a point of contact to patients assisting with support throughout the multidisciplinary nature of cancer treatment. The navigator works in alignment with the nurse navigation department, assisting the team in areas that may include illness perception, care coordination, and appointment assistance. The Patient Care Navigator must demonstrate a model of service, team collaborator, and must be willing to interact with other co-workers and patient's caregivers. This role includes staying current with organizational resources and available community resources for the patient population the organization serves.
Education
High School Diploma or Equivalent Required
Bachelor's Degree Preferred
Other Completion of Patient Care Navigator education program preferably from George Washington University or American Cancer Society Preferred
Experience
2 years experience in a healthcare setting or equivalent preferably in oncology Required
General Experience and knowledge of basic clinical concepts Required
General Experience and knowledge of EPIC systems
General Experience and knowledge of complex patient issues and social determinants of health Required
General Experience and knowledge of patient outreach methods and strategies Preferred
General Experience with medical terminology Preferred
General Experience working as a patient navigator/community health worker Preferred
